Squamish Unattached Patient Registry

Are you looking for a family doctor in Squamish?
Then please sign up for the Unattached Patient Registry!

  1. Signing up for this service requires only your name, and a cel phone number.  No personal information beyond that – medical or otherwise – will be collected.
  2. They may communicate once – to validate your registration – but then won’t text again until they have a provider available, or to possibly confirm that you’re still in need of a GP.   They won’t spam you.
  3. In the meantime, the local Walk in Clinic is the right place to start when seeking non-emergency/primary care.

We are still offering physician appointments!
To avoid any spread of COVID we are trying to triage as many appointments as possible to scheduled phone or video appointments.
If needed we are still offering scheduled in-office appointments.
Please call the office to schedule an appointment (phone, video or in-office).

As of November 4th, 2020 YOU MUST WEAR A MEDICAL MASK WHEN WITHIN THE SMC BUILDING. If you arrive at the clinic without protection you will need to re-schedule your appointment. We will be selling medical masks for $2 each. This mask mandate is from BCCDC.  Thank you for your understanding during this difficult time.

HOURS OF OPERATION

We are open for appointments 0900 to 1630h Monday to Friday.
We are closed all statutory holidays.
Some of our physicians work early morning evening clinics — please call for details.

As of June 1, 2021, we are NOT accepting new patients.
(check back regularly as this may change)

INTERNAL MEDICINE SPECIALIST

Dr. Yasha Tashakkor is an Internal Medicine specialist.
For consults or cardiac (treadmill) stress testing in our office facility physician’s offices are to please fax referrals to 604-914-2524.
For Holter Monitors patients are to please call or drop into our office as we can often provide same day or next day service.
We are only offering urgent ECGs to non-SMC patients at this time.
Non-SMC patients are to have their non-urgent ECGs done at SGH.

GENERAL PEDIATRICIAN

Dr. Jennifer Carlisle is a General Pediatrician seeing patients in our office.
Referring physicians are to please fax referrals to our office 604-892-3545.

Neonatologist / Pediatrician

Dr. Rebecca Sherlock is seeing infants under one year of age at SMC.
Physician’s offices are to please FAX referrals to our office at 604-892-3545.

Obstetrician & Gynecologist

Dr. Hema Krishnan ObGyne is now seeing patients at SMC.
Physician’s offices are to please fax referrals to 778-605-2509.

UBC Faculty of Medicine Teaching Facility

The SMC is a UBC Faculty of Medicine teaching facility, and there will at times be resident physicians and/or nurse practitioners training with our physicians.  All services these physicians or nurses provide will be directly overseen by one of our physicians.
